The program is designed to be simple, transparent, and limited to dental services performed at The Mill Creek Dentist.

Pay $540 for your adult preventive care package, or request an in-house payment arrangement of 12 monthly payments of $45.
Receive Your Vouchers
After enrollment, you receive courtesy vouchers that may be redeemed during the 12-month voucher period.
Use Savings at Our Office
Redeem vouchers for eligible preventive services and savings on additional dental treatment performed at our office.
Treatment Savings
Adult package patients receive savings vouchers that expire 12 months from the purchase date.
Up to 15% Savings
Savings vouchers may be applied toward eligible dental treatment beyond preventive care, up to the equivalent of 15% off the treatment fee.
Valid for 12 Months
All vouchers expire 12 months from the package purchase date and have no cash value.

Vouchers are valid only for dental services performed at The Mill Creek Dentist and may not be used with outside providers.

No Insurance Billing
This program is intended for patients without dental insurance and may not be combined with dental insurance, Medicaid, Medicare, or other discount programs.
Voucher Expiration
All vouchers expire 12 months from the package purchase date. Unused vouchers do not roll over and have no cash value.
Non-Transferable
Package benefits and vouchers apply only to the enrolled patient and may not be transferred to another person.
Installment Payments
The optional monthly payment arrangement is an in-house payment plan for the $540 package purchase price. It is not a monthly insurance premium.
Missed or Failed Payments
If installment payments are missed or declined, voucher redemption may be paused until the account is brought current. The patient remains responsible for the package purchase price and any services received.
Cancellation
If a patient cancels the payment arrangement before the $540 package purchase price is paid in full, the patient is responsible for the full retail value of services and vouchers already used, less payments made.
Clinical Limitations
Cleanings included in this program are adult prophylaxis visits. Periodontal therapy, scaling and root planing, and periodontal maintenance are not included.

What if I need a deep cleaning?
